Apr 2016 - Shapeshift Exchange Hack - $200k (Switzerland)

"Since the investigation into the ShapeShift hack last week started, we had suspicion that someone previously on the team was involved, and that this person assisted an outside hacker. We are confident now that is is indeed the case. The story continues to unfold, and evidence continues to be revealed. We have been working with a forensic specialist from LedgerLabs, who has been terrific. A civil suit is ongoing, as are multiple criminal investigations of the perpetrators."

How could this happen at the "safest asset exchange on Earth"? Let’s ask Bob, the disgruntled employee who apparently emptied out the hot wallet and installed a backdoor on another employee’s computer. Then, he found some other hacker named Rovion who apparently was willing to up and buy the access to for 50 BTC. Enabling the wallet to be emptied a total of 3 times. The official story, if believed, stinks of stupidity and carelessness, and there is any number of easy steps that could have been taken to avoid it in the first place.
